Community Rehabilitation Team and Rapid Assessment Link (RAL)

The Community Rehabilitation Team (CRT) provides a home-based, multi-disciplinary service for older people and adults with a disability at home. It gives advice about managing disabilities on a long term basis, and what service users can do to try to avoid unnecessary admission to hospital/long-term care. The team works closely with other agencies, health and social care departments and organisations to ensure that patient care is coordinated. Anyone who is eligible will be assessed by the team to determine the correct help and support. There is a Rapid Assessment element available within the Service to provide rapid assessment and support to those at risk of hospital admission. Who is eligible
  • People over 16 years with complex physical health needs;
  • Those with a physical or functional impairment which has potential for improvement through rehabilitation;
  • The person is able to be maintained safely within the home setting.
How to access the service The team accepts self-referrals through the phone number: 0141 232 8213, as well as referrals from other agencies or health/social work departments. They will then arrange for an assessment to be carried out and treatment to follow as required.
